We are seeking a skilled Physical Therapist to join our team at Rehab Without Walls Neuro Rehabilitation. As a Physical Therapist, you will have the opportunity to make a meaningful impact in the lives of our patients by providing functional rehabilitation in their natural home and community environment.
About the RoleAs a Physical Therapist, you will be responsible for performing evaluations and developing treatment plans that cater to the unique needs of our patients. You will work closely with our team of dynamic therapists to ensure seamless collaboration and coordination of care. Your expertise will be invaluable in helping our patients regain functional capabilities and independence.
Responsibilities- Perform evaluations and develop treatment plans that address the complex needs of our patients
- Regularly re-assess the effectiveness of treatment plans and attend staffing meetings and family conferences
- Perform discharge evaluations and provide recommendations for home and community safety
- Communicate patient needs and progress to the treatment team, physician, and family members
- Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes
- Minimum of bachelor's degree in Physical Therapy from an accredited program
- Current, unrestricted license as a Physical Therapist by the state in which practicing
- Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations
- A minimum of one year's work experience as a physical therapist preferred
- Demonstrates knowledge of rehabilitation techniques related to complex neurological injury
- Communicates effectively and professionally in verbal and written interactions
