Responsibilities:

  • Develop and implement individualized care plans for patients with chronic wounds, working collaboratively with healthcare providers and other members of the patient's care team.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for improvement and developing strategies to address complex challenges.
  • Provide education and support to patients and their caregivers, empowering them to manage their care and make informed decisions.
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
  • Participate in ongoing professional development and training, staying current on best practices and advancements in wound care.

Requirements:

  • Certification through ANCC (American Nurses Credentialing Center) or AANP as a Nurse Practitioner.
  • WOCN (Wound, Ostomy and Continence Nurses) certification or other company-approved wound care certification required.
  • Minimum 3 years' experience treating chronic wounds in a hospital, outpatient, or post-acute setting.
  • Demonstrated ability to work independently and collaboratively as part of a multidisciplinary team.
