About the Role
The Surgical Care Manager RN will collaborate with the clinical team and medical provider to provide discharge planning coordination and intervention for patients with complex health needs. This role will promote patient wellness, improved care outcomes, and efficient utilization of health services.
This position will be dedicated to the surgery team.
Key Responsibilities
- Assess patients' care coordination and discharge planning needs through initial and ongoing assessments.
- Create focused, anticipated discharge plans for high-risk patients with identified needs.
- Collaborate with the clinical team and medical provider to initiate discharge plans for patients.
- Stay abreast of changing organizational, emergency department, Care Management, and clinical trends, regulatory matters, and third-party payer requirements related to clinical care, discharge planning, and precertification or aftercare benefits.
Requirements and Qualifications
- BSN preferred; ADN required
- 3-5 years related experience; at least 2 years in CM with Surgery or ICU preferred
- Maryland Registered Nurse License
- Intent to achieve MD licensure if out of state
- Case Mgmt cert preferred
