Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at ECHN. As a key member of our healthcare team, you will be responsible for providing high-quality patient-centered care, collaborating with interdisciplinary teams, and promoting positive patient outcomes.

Key Responsibilities
  • Conduct comprehensive assessments and re-assessments of patient conditions and care needs, including psychosocial, physical, educational, and cultural aspects.
  • Develop and revise individualized treatment plans based on patient responses to treatment and daily evaluations.
  • Provide education and support to patients and families regarding pain management, medication administration, and other relevant health topics.
  • Collaborate with treatment teams to effectively meet patient treatment needs and promote a safe and therapeutic environment.
  • Participate in early response codes and Dr. Strong codes, and attend PMT refresher trainings as required.
Requirements
  • Graduate of an accredited school of nursing with a current Registered Nurse license in the state of Connecticut.
  • Certification in Cardio Pulmonary Resuscitation (CPR) upon hire and maintenance of clinical competency during employment.
  • Training in PMT and de-escalation techniques, with certification within 6 months of hire.
  • Basic computer skills and ability to communicate effectively in English, both verbally and in writing.
Preferred Qualifications
  • Experience in psychiatric nursing and patient care coordination.
  • Knowledge of medications, including psychotropic drugs, and their correct administration based on patient age and clinical condition.
  • Ability to work autonomously and as part of a team, with strong critical thinking and problem-solving skills.
