ClearSky Health is seeking a skilled Licensed Practical Nurse to join our team as a Patient Care Specialist. As a key member of our healthcare team, you will provide high-quality nursing care to patients, under the direction of a Registered Nurse and in accordance with physician's orders and established standards and protocols.
Key Responsibilities- Provide skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
- Implement orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
- Provide information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
- Participate in evaluation of the effectiveness of interventions in relation to patient responses and seek guidance in redirection when necessary.
- Report to supervisor on patient care rendered, problems concerning patients' needs, and resolution to those needs.
- Help identify patient problems and potential risks and contribute to the development of interventions.
- Assist in identification of patient's learning needs and reinforce self-care and other techniques.
- Appropriately designate applicable charges to individual patients using the hospital's established system.
- May be required to work during inclement weather and other staffing emergencies.
- Provide an environment conducive to safety for patients, visitors, and staff. Assess the risks for safety and implement appropriate precautions. Comply with appropriate and approved safety and Infection Prevention standards.
- Maintain regular attendance in keeping with assigned work schedule.
- Perform other duties as assigned to support overall effectiveness of the organization.
- Two years' experience in inpatient hospital setting, inpatient rehabilitation experience preferred.
- Current state license as a Licensed Practical Nurse required.
- Current CPR certification required.
- A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.
- Both gross and precise motor functions.
- Lifting/exerting of up to 50 lbs.
- Possible exposure to bodily fluids.
- Visual acuity required for patient assessment and documentation of care.
- Acute hearing required for accurate patient assessment.
- Sufficient manual dexterity to operate equipment and computer keyboard.
- Close vision and the ability to adjust focus.
