  • Collaborate with healthcare professionals to develop and implement patient care plans that meet individual needs and promote optimal health outcomes.
  • Conduct thorough medication reviews to identify potential interactions, allergies, and other safety concerns.
  • Develop and maintain accurate and up-to-date patient records, including medication lists and allergy information.
  • Stay current with the latest developments in pharmacy practice, including new medications, treatments, and technologies.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • PharmD degree or equivalent from an accredited institution.
  • Current licensure as a pharmacist in the state of practice.
  • Completion of an ACPE-accredited immunization training program.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Strong problem-solving and critical thinking skills.
