- Develop and implement individualized treatment plans based on evaluation results and functional goals.
- Monitor and evaluate patient outcomes, adjusting treatment plans as needed, and provide status updates to physicians.
- Identify factors affecting patient comfort and address these needs.
- Plan for and complete patient discharge, including providing written home programs, patient/family teaching, and referrals for community services.
- Complete required documentation, including patient progress notes, team conference reports, and discharge summaries.
- Apply insurance guidelines regarding authorization and number of visits.
- Perform manual techniques, patient positioning, transferring, and transporting duties, utilizing proper technique.
- Demonstrate skill in using modalities, exercise equipment, and other devices available in the area.
- Participate in performance improvement activities to identify and resolve problems affecting patient care outcomes and improve quality of care.
- Manage assigned caseload, informing site leadership of coverage needs and ability to cover, and assist with coverage based on department needs and regional needs.
- Pursue professional growth through continuing education courses, department inservices, and reading professional literature.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) within 6 months.
- Physical Therapist license issued by the state of Wisconsin.
- Bachelor's Degree in Physical Therapy.
- No experience required.
