We are seeking a highly skilled Clinical Staff Pharmacist to join our team at ChristianaCare's Newark campus. As a key member of our in-patient pharmacy team, you will play a critical role in ensuring the safe and effective delivery of medications to our patients.
Key Responsibilities
- Review and interpret provider medication orders, evaluating drug efficacy, therapeutic goals, and indications, as well as reviewing for contraindications and precautions.
- Resolve medication-related problems in consultation with prescribers and nurses, clarifying questioned orders prior to dispensing the first dose.
- Evaluate specific medication therapies consistent with departmental policy, including anticoagulation, aminoglycosides, and vancomycin.
- Appropriately convert and manage medications per P&T-approved pharmacist policy, including IV to PO and renal dose adjustment.
- Report, record, assess, and evaluate adverse drug reactions.
- Provide accurate, timely, and cost-effective drug information to medical, pharmacy, nursing, and other healthcare professionals.
- Coordinate and supervise workplace activities and resources, promoting professional competence and ensuring services rendered and work performed meet specific standards.
Requirements
- Bachelor of Science degree in Pharmacy or PharmD degree from an accredited College of Pharmacy required.
- At least one year of experience working as a pharmacist in an acute care hospital, or two years of previous experience working as a Pharmacy Intern or Pharmacy Technician in an acute care hospital, or two years of experience as a pharmacist in an in-patient hospital other than acute care, a long-term care setting, or an out-patient Infusion Center.
- Current licensure (or eligible to reciprocate) in the State of Delaware.
