Key Responsibilities:

  • Conduct comprehensive assessments of patient and family needs to identify outcomes and develop personalized care plans.
  • Maintain up-to-date education and competencies to ensure proficiency in providing high-quality patient care.
  • Apply evidence-based nursing practices to develop care plans that meet current statutes, rules, and regulations.
  • Collaborate with patients and families to establish realistic and measurable expected outcomes based on nursing diagnoses, patient capabilities, goals, and available resources.
  • Coordinate and delegate care to ensure safe and effective delivery.
  • Fulfill all organizational requirements and take on supplemental roles as needed, such as Charge Nurse or Preceptor.

Requirements:

  • Associate's or Vocational degree in Nursing
  • 0-2 years RN experience
  • Licensed Registered Nurse - MN Board of Nursing required
  • BLS Tier 1 - Basic Life Support - Multisource required
  • Valid Driver's License required

Preferred Qualifications:

  • BSN
  • May consider new graduate RN
  • 0-2 years homecare/hospice experience
  • CHPN Certified Hospice and Palliative Nursing
  • RN-BC RN Case Management
  • ANCC-RNCM Certification in Case Management

Physical Demands:

  • Medium Work:
  • Lifting weight up to 32 lbs. occasionally
  • Lifting weight up to 25 lbs. frequently
