Key Responsibilities
  • Direct Patient Care: Provide skilled nursing care to patients in their homes, under the supervision of a registered nurse.
  • Plan of Care Implementation: Implement the plan of care initiated by the registered nurse, ensuring that patients receive the best possible care.
  • Documentation and Communication: Accurately and timely document patient information, and communicate effectively with patients, families, and healthcare team members.
  • Patient Education: Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Requirements
  • Current CA Vocational Nurse License: Possess a current and active license to practice as a vocational nurse in California.
  • Experience and Qualifications: One year of experience as an LVN, with pediatric experience preferred. New graduates may be considered under the Flex Waiver Program.
  • Certifications and Clearances: Current PPD or chest x-ray (if applicable), CPR certification, and a valid driver's license (for local travel).
