Job Summary

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to provide high-quality nursing care to patients in their homes. As a key member of our team, you will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians' orders.

Key Responsibilities
  • Deliver and Coordinate Quality Patient Care: Provide skilled nursing care to patients in their homes, ensuring that their needs are met and their care plans are implemented.
  • Assess and Report Patient Condition: Continuously observe and assess patient condition and care needs, reporting changes in condition to the supervisor and/or physician as appropriate.
  • Document Patient Care: Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in Nursing Care Planning: Participate in the development and implementation of nursing care plans, ensuring that they are tailored to meet the unique needs of each patient.
  • Provide Education and Support: Educate patients and their families regarding disease processes, self-care techniques, and prevention strategies, and provide emotional support and guidance as needed.
Requirements
  • Education and Licensure: Graduate of an accredited school of nursing and current, unrestricted state license as a Licensed Nurse in the state of practice.
  • CPR Certification: Current CPR certification.
  • Experience and Skills: Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures. One year of experience in pediatrics or a related field is preferred.
Preferred Qualifications
  • Home Health Experience: Previous experience in home health care is highly desirable.
  • Strong Communication Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Physical Requirements
  • Physical Demands: Must be able to speak, write, read, and understand English. Must be able to travel and lift 50 pounds. Must be able to sit, climb stairs, and move equipment without assistance.
Work Environment
  • Work Setting: Must be able to function in a wide variety of environments, including patients' homes, and be exposed to allergens and other conditions.
