About the Role

The University of Maryland Medical System is seeking a highly skilled Transitional Care Nurse Specialist to join our team in the Comprehensive CARE Center. This individual will play a critical role in working with patients who require transitional care, assessing their complex needs, and developing personalized care plans.

As a key member of our multidisciplinary team, you will conduct thorough assessments, identify barriers to healthcare, and provide telephonic guidance, advice, and support to patients and families. Your expertise in case management will enable you to facilitate patient access to community resources, promote self-management, and improve clinical outcomes.

We are looking for an experienced nurse with a strong background in case management, who can effectively communicate with patients, families, and healthcare providers to ensure safe and efficient services. If you possess excellent critical thinking skills, are able to work independently, and have a passion for promoting patient self-management, we encourage you to apply.

Key Responsibilities:

  • Conduct thorough assessments of patients' medical, physical, social, emotional, psychological, and financial needs
  • Develop and implement personalized care plans to address patients' complex needs
  • Provide telephonic guidance, advice, and support to patients and families
  • Facilitate patient access to community resources
  • Maintain accurate and timely documentation of actions/services in the EMR

Requirements:

  • Current Maryland RN license required
  • Completion of a Bachelor's of Science degree in Nursing preferred
  • Three (3) years nursing care experience required, with case management experience preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team

Estimated Salary Range: $51,000 - $72,000 per year
