Job Summary

We are seeking a compassionate and skilled Certified Home Health Aide/C.N.A. to join our team at Trinity Health At Home. As a Contingent Hospice CNA, you will provide one-to-one care to our patients in their homes, following physician orders and in compliance with state regulations and our policies and procedures.

Responsibilities

• Follows care plan developed by RN
• Reports client changes in condition to RN, as described on care plan
• Assists clients with personal care activities, including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities
• Assists clients with household tasks, including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding
• Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered
• Performs range of motion and other simple procedures as an extension of therapy services as assigned
• Assists clients in self-administration of medication, as assigned by an RN and documented in the clinical record

Requirements

• High School diploma or GED required
• Completion of CNA 75 hr course
• Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program
• Documentation of one year's employment as an aide in an institution, home care, or hospice setting
• Meets agency hiring criteria as defined in agency minimum standards
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family
• Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis
• Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program
• Must have current Driver's license and reliable transportation to and from work site
