ClinDCast is seeking a skilled Epic Healthy Planet Analyst to join our team. The ideal candidate will have a strong understanding of population health management principles and practices, as well as experience with Epic Systems, specifically the Healthy Planet module.

Key Responsibilities:

  • Develop and implement population health management strategies using Epic Healthy Planet
  • Collaborate with healthcare organizations to design and implement effective care management workflows
  • Provide training and support to end-users on Epic Healthy Planet
  • Stay up-to-date with industry trends and best practices in population health management

Requirements:

  • Bachelor's degree in healthcare management, information technology, or a related field
  • Experience with Epic Systems, specifically the Healthy Planet module
  • Strong understanding of population health management principles and practices
  • Certification in Epic Healthy Planet
