Job Description

Position: Site Medical Practitioner

Locations: Various locations available

Schedule: 8:00 AM - 5:00 PM

Weekly Hours: 40

Role Overview:

Under the guidance of the Chief Medical Officer, the Site Medical Practitioner is tasked with overseeing the professional, organizational, and administrative facets of delivering exceptional medical care to patients.

Key Responsibilities:

  • Lead the medical care and treatment of patients while supervising the interdisciplinary care team.
  • Collaborate with nurse practitioners and community-based primary care providers for shared patient populations.
  • Identify and implement strategies to enhance the quality, cost-effectiveness, and utilization of care.
  • Manage a patient caseload as the primary care physician and engage in direct medical services.
  • Participate in multidisciplinary team activities, including case discussions and family meetings.
  • Contribute to the Utilization Review of medical services provided.
  • Develop and oversee ambulatory medical services.
  • Act as a liaison with other medical institutions and build relationships with specialists for coordinated care planning.
  • Facilitate medical education initiatives for staff and trainees.
  • Conduct patient assessments and treatments in various settings.
  • Serve as a resource for clinical staff within the facility.
  • Engage in staff development and clinical training for rotating trainees.
  • Participate in preventive health programs and quality assurance initiatives.
  • Perform additional duties as assigned.

Qualifications:

  • Graduate of an accredited medical school.
  • Board Certification in Internal Medicine or Family Practice.
  • Experience or certification in Geriatric Medicine is preferred.
  • Completion of residency in Internal Medicine or Family Practice is required.
  • Current active unrestricted medical license in New York State.
  • Excellent communication skills, both verbal and written.
  • Minimum of one year of experience with elderly or frail populations is preferred.
  • Must have Medicaid and Medicare Provider IDs in good standing.
