Position Summary:The official title for this role is Lead Patient Care Specialist. The Lead Patient Care Specialist (LPCS) is responsible for creating personalized care strategies, coordinating member services, and working closely with members and their families to ensure they receive the necessary medical treatment and support. The LPCS will engage with members to assess their needs, formulate tailored action plans, and collaborate with care teams to enhance the member experience. Key responsibilities include providing emotional support, addressing administrative challenges, and ensuring timely access to care. Compassion, healthcare expertise, and outstanding customer service are vital for helping members achieve health independence.
Key Responsibilities:- Conduct interviews with members to evaluate their medical and social needs while providing education regarding their health conditions and medications, and developing customized care plans.
- Address member inquiries and concerns, ensuring compliance with healthcare regulations.
- Collaborate with multidisciplinary teams, identify medical and social resources, and coordinate social service initiatives.
- Advocate for members by consulting with healthcare providers, scheduling appointments and treatment plans, monitoring member progress, and assisting with healthcare challenges.
- Maintain empathy and professionalism in all communications with members and their families.
- Support coordination of behavioral health services, substance abuse resources, and community support.
- Perform additional duties as assigned.
- Possession of a valid driver's license and vehicle insurance.
- High school diploma or GED is required.
- A minimum of 1 year of experience in case management, member care, customer service, or related fields is required.
- Medical Assistant certification or knowledge of medical terminology is preferred.
- Exceptional communication, interpersonal, customer service, and organizational skills.
- Proficient computer skills for documentation and communication.
- Ability to work independently and collaboratively within a team to deliver member care.
- Strong multitasking, organizational, and attention to detail skills are essential for managing care for multiple members simultaneously.
- Demonstrated ability to utilize resources effectively for problem-solving while maintaining composure in challenging situations.
- Proficient speaking, reading, and writing skills are required, along with strong proofreading abilities.
- Bilingual proficiency in Spanish is a requirement.
- Ability to travel using a personal vehicle for member visits.
- Prolonged periods of sitting at a desk and using a computer.
- Able to lift up to 15 lbs.
