Responsibilities- Implement and monitor patient care plans, ensuring that patients receive high-quality care and meet their individual needs.
- Coordinate the care of other healthcare professionals, including physicians, nurses, and other support staff, to ensure seamless communication and collaboration.
- Assess and coordinate patient discharge planning needs, ensuring that patients receive the necessary support and resources to transition smoothly to their next level of care.
- Provide education and support to patients and their families, empowering them to take an active role in their care and make informed decisions.
- Collaborate with the healthcare team to develop and implement policies and procedures that promote patient safety and quality care.
- BLS Provider certification within 1 month of hire date.
- Certification in Neonatal Resuscitation within 6 months of hire date.
- Certification in The S.T.A.B.L.E Program within 6 months of hire date.
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition Compact State within 2 months of hire date.
