Mosaik Medical is seeking a highly skilled Clinical Pharmacy Manager to join our team. As a Clinical Pharmacy Manager, you will be responsible for leading our clinical pharmacy services, promoting evidence-based practice, and developing new pharmacy practice programs.
Key Responsibilities:- Develop and implement policies and procedures to improve medication safety and meet professional standards.
- Partner with the Pharmacy Services Director to establish and implement short and long-term plans to meet clinical, strategic, and financial targets.
- Oversee clinical pharmacy services, including understanding roles, workflows, clinic schedules, and support staffing coverage.
- Collaborate with operations and clinical leaders to support effective and efficient clinical pharmacy services.
- Monitor and communicate key performance indicators and balanced scorecard measures to the clinical pharmacy services team.
- Provide support to clinic leadership to ensure timely resolution of critical incidents, patient grievances, and patient complaints.
- Foster a culture of continuous improvement and standardization using change and process improvement methods.
- Manage the performance of the team, conduct monthly meetups, and provide real-time and consistent coaching with direct reports.
- Ensure accountability of the team, including adherence to policies, procedures, guidelines, workflows, and addressing attendance, performance, and behavior concerns.
- Coordinate medication therapy management (MTM) and collaborate with PCPs in managing medications using evidence-based recommendations.
- Assess, implement, and monitor medication-related therapies, detecting therapeutic incompatibilities and assessing medication adherence.
- Effectively utilize team-based care models by leveraging expertise of all members of the multi-disciplinary team.
- Participate in the Controlled Medication Oversight Committee (CMOC) and work with clinical leadership to ensure safe prescribing practices of controlled medications.
- Educate clinical teams about drug information and medication selection, and provide consultation and education to clinical staff.
- Maintain accurate electronic health records of patients and complete documentation in a timely manner.
- Develop and maintain residency program accreditation through the American Society of Health-System Pharmacists (ASHP).
- Establish and maintain policies that protect residents and the organization, facilitate resident training, and promote a learning environment consistent with national accreditation standards and regulatory requirements.
