MDS Nurse Specialist

CareSpring, a leading healthcare provider, is seeking an experienced MDS Nurse Specialist to join our team in Loveland, OH. The successful candidate will be responsible for assessing new and existing patients to ensure their care needs are addressed and documented on the Minimum Data Set (MDS).
Salary Range: $29.50 - $44 Per Hour

We require an Admissions/MDS Nurse to oversee the MDS process following the RAI manual, work with other MDS nurses to complete initial assessments for new admissions, and properly manage patient admissions scheduling, MDS assessment & care planning.
You will assess new and existing patients to ensure that all of their care needs are addressed and documented on the MDS.
Your Responsibilities:
- Oversee the MDS process following the RAI manual
- Work with other MDS nurses to complete initial assessments for new admissions
- Properly manage patient admissions scheduling, MDS assessment & care planning
- Complete required submissions weekly or as directed by the DON
- Review validation report to ensure error-free submission
- Obtain & review QM's
- Work with IDT to improve resident outcomes
- Attend IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process
- Address the needs of residents by developing & updating comprehensive & nurse aide care plans
- Ensure non-nursing sections of MDS & CAA's accurately reflect resident needs
- Monitor care plans to identify resident needs
- Review order recap reports & nurses' notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed
- Monitor residents' change of status & determine need for change per RAI
- Ensure accuracy of supportive documentation according to MDS/RAI regulations
- Work with facility consultants to implement recommended changes
- Complete AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperwork
Your Qualifications:
- A current LPN or RN license in the state of Ohio
- A flexible work schedule & the ability to work independently
- An understanding of the MDS process & a willingness to continually learn
- Knowledge in PDPM & RUG
