Job Opportunity: Physical Therapist - Home Health Specialist

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Specialist. As a Home Health Physical Therapist, you will be responsible for providing rehabilitative care and skilled nursing to clients in their homes.

Responsibilities:

  • Conduct home visits to clients in designated geographic territories to assess and develop individualized care plans.
  • Perform diagnostic tests and measurements to evaluate client mobility, strength, and balance.
  • Implement evidence-based interventions to promote wound repair and prevent wound formation.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve client mobility and function.
  • Continually assess and revise the care plan to ensure optimal client outcomes.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document client observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Qualifications:

  • A current Physical Therapist license in the state of practice.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
