We are seeking a skilled Physical Therapist Assistant to provide high-quality care to our home care clients in Tennessee.
Key Responsibilities:- Provide physical therapy services and education to clients in their homes, following the physician's Plan of Care and therapy care plan.
- Document all services provided, observations, client progress, and recommendations on designated forms and submit within seven days of providing service.
- Report changes, concerns, or schedule changes to the therapist and/or nursing supervisor on the day of the observed change.
- Participate in interdisciplinary activities and attend client care conferences as needed.
- Promote referrals to other disciplines as appropriate and attend staff meetings and continuing education programs.
- Current license, registration, or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the physical therapy examination.
- Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
- Minimum of six months experience as a Physical Therapy Assistant.
- Demonstrated written, verbal communication skills, and interpersonal skills.
- Licensed driver with a clean driving record and insured in accordance with state and/or Agency requirements.
- Competitive hourly rate of $36.00, plus shift differentials where applicable.
