At CareOne, we are seeking a Certified Home Health Aide (CHHA) to join our team in an Assisted Living setting. As a CHHA, you will play a vital role in providing high-quality care to our residents, ensuring their comfort, dignity, and well-being.

Responsibilities:
  • Provides or assists in Activities of Daily Living (ADLs), including personal hygiene, grooming, and skin care.
  • Assists with toileting, positioning, and transfer activities, as well as frequent turning and positioning of bed-bound residents.
  • Assists with prescribed exercise and Range of Motion (ROM) activities, as taught by the Hospice Nurse.
  • Assists with light housekeeping, including changing bed linen and patient laundry.
  • Feeds and assists with meals.
  • Obtains temperature, pulse, and respirations as supervised by the Hospice Nurse.
  • Reports any changes in resident condition to the Hospice Nurse and documents information on the Home Health Aide Certification of House form.
  • Assists or reminds patients to take medication, in compliance with regulatory and accrediting bodies.
Requirements:
  • High School Diploma or equivalent.
  • State Certified CHHA license.
  • Experience in hospice or home health, preferred.
  • No experience necessary, but 1 year preferred.
  • Must be willing to travel.
  • Must have a valid driver's license and reliable transportation.
