Our mission is to treat the entire individual rather than merely addressing symptoms, utilizing a collaborative interdisciplinary team (IDT) approach to assess and manage participant care.
Role Overview:
Under the guidance of the Medical Director, the Advanced Practice Nurse collaborates closely with WelbeHealth Physicians and the IDT to deliver thorough assessments, care planning, and primary healthcare services to our participants.
Key Responsibilities:
- Conduct in-person comprehensive evaluations, including initial, semi-annual, and unscheduled assessments, ensuring continuous medical support for participants.
- Work alongside physicians and the IDT to formulate individualized care plans for each participant.
- Assist in discharge planning from both acute and long-term care settings in collaboration with the medical team.
- Provide ongoing care for participants residing in nursing facilities, making regular visits based on individual needs.
- Coordinate with WelbeHealth physicians, community-based PACE physicians, and the IDT to ensure effective care management.
- Review clinical outcomes and utilization data, including hospital admissions, to identify best practices and enhance policy frameworks.
- Participate in a rotating schedule for night and weekend on-call duties.
- Graduation from an accredited nursing program.
- Certification as a Nurse Practitioner with Prescriptive Authority.
- Active RN State License.
- A minimum of two years of experience working with the elderly population.
- Current understanding of chronic care and geriatric best practices.
- Access to reliable transportation.
