Job Summary

Key Responsibilities
  • Collaborate with Healthcare Team: Work closely with registered nurses and other clinicians to deliver care according to the service plan.
  • Companionship and Support: Provide companionship and personalized care, encouraging independence and self-care in clients.
  • Activities of Daily Living (ADL) Assistance: Assist clients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
  • Medication Support: Assist clients with the self-administration of medication as needed, ensuring safety and adherence to protocols.
  • Mobility Assistance: Provide mobility and transfer assistance within guidelines, promoting patient safety and comfort.
  • Meal Preparation and Light Housekeeping: Prepare meals and snacks for clients, which may include grocery shopping according to dietary needs and preferences, and perform light housekeeping duties to maintain a clean and comfortable environment.
  • Incontinence Care and Transportation Support: Assist with incontinence care, ensuring client dignity and hygiene, and assist with transportation needs for appointments, errands, or outings.
  • Compliance and Documentation: Remain in compliance with HIPAA guidelines and all state and federal regulations, and document clinical notes clearly and concisely.
  • Equipment Management: Set up and maintain medical equipment and supplies, including Durable Medical Equipment (DME), ensuring readiness and functionality.
Requirements
  • CNA License: Valid CNA certification in the State of Pennsylvania.
  • Experience: Experience in hospice or palliative care preferred.
  • Clear Background Check and Drug Screening: Clear background check and drug screening.
  • Strong Communication and Empathy Skills: Strong communication and empathy skills.
  • Self-Starter and Team Player: Self-starter, able to work independently and collaboratively.
  • Physical Ability: Physical ability to perform job duties.
  • Reliable Transportation: Reliable transportation and a valid driver's license.
