We are seeking a highly skilled Registered Nurse to join our team as a Case Manager in the Emergency Department. Key Responsibilities- Collaborate with healthcare providers to develop and implement effective care plans
- Monitor patient progress and make adjustments to care plans as needed
- Ensure compliance with nationally recognized medical utilization criteria, HCFA/OBRA, and state of Connecticut regulatory requirements
- Provide education and support to patients and their families to promote health and well-being
- Bachelor's degree in Nursing (BSN)
- Current licensure as a Registered Nurse in Connecticut
- Five years of diversified, progressive experience in acute care and/or other settings within the continuum of care
- BLS certification required
