About the Job:

The Nurse Clinical Care Coordinator at UPMC Your Care plays a critical role in delivering high-quality patient care and ensuring seamless transitions between healthcare facilities. This position requires a registered nurse (RN) to work with patients in various locations across Pittsburgh and Monroeville areas, developing comprehensive care plans and collaborating with healthcare providers to achieve optimal outcomes. Key responsibilities include:

• Providing direct patient care, conducting comprehensive face-to-face assessments, and identifying gaps in care based on clinical standards.
• Developing and modifying care plans to promote symptom management, healthy lifestyles, and reduce unnecessary ER utilization and hospital admissions/readmissions.
• Collaborating with healthcare providers, caregivers, and community resources to achieve optimal patient outcomes.
• Utilizing computer systems to document patient information and track progress.

Our Ideal Candidate:

We seek a highly motivated and experienced Nurse Clinical Care Coordinator who can deliver high-quality patient care and work effectively in a fast-paced environment. To succeed in this role, you must have a current licensure as a registered nurse (RN), one year of health insurance experience, and excellent organizational and communication skills. Basic life support (BLS) certification and computer proficiency are also required. We offer a competitive salary range of $65,000 - $85,000 per annum, depending on education and nursing experience.
