Care Planning Coordinator Opportunity

We are seeking a dedicated Registered Nurse (RN) to join our team at Cottonwood Healthcare and Rehabilitation Center in Laramie, WY. As a Care Planning Coordinator, you will play a vital role in coordinating the facility's Medicare, Resident Assessment Instrument (MDS), and care planning process.

Responsibilities:
  • Coordinate the facility's Medicare and MDS processes
  • Develop and implement care plans for residents
  • Collaborate with healthcare teams to ensure high-quality care
  • Monitor and evaluate resident outcomes
Requirements:
  • Graduate of an accredited nursing program
  • Valid, unrestricted RN license in the state
  • Previous MDS experience preferred
  • Previous experience in Long Term Care preferred, but not required
