Job Summary

The Care Coordination Specialist will be responsible for assessing, planning, and implementing care management activities that promote quality, cost-effective healthcare outcomes. You will develop personalized care plans, educate members and their families on services and benefit options, and collaborate with healthcare providers to ensure optimal care delivery.

Key Responsibilities
  • Assess member needs: Conduct thorough assessments to identify member requirements and develop comprehensive care plans.
  • Develop care plans: Collaborate with healthcare providers to create personalized care plans that address member needs and goals.
  • Coordinate care delivery: Work closely with healthcare providers to ensure timely and effective care delivery.
  • Educate members and families: Provide education on disease processes, care options, referrals, and healthcare benefits to empower members and their families.
  • Collaborate with leadership: Partner with leadership to improve and enhance care delivery and quality metrics.
Requirements

To succeed in this role, you will need:

  • A Degree from an Accredited School or Nursing or a Bachelor's degree in Nursing and 4 - 6 years of related experience.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
What We Offer
