Job Summary:

As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to our most vulnerable populations in the Fox Valley and Brown counties, WI area.

Key Responsibilities:

  • Conduct comprehensive assessments of patients in assisted living and long-term care facilities
  • Manage patients with acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Provide short-term sub-acute rehabilitation care and ensure accurate and complete documentation
  • Communicate and collaborate with the interdisciplinary care team to ensure seamless care transitions
  • Conduct advanced illness and advanced care planning conversations with patients and caregivers
  • Provide education and counseling to patients and caregivers on disease management and self-care

Requirements:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state of practice or ability to obtain by start date
  • Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
  • 1+ years of clinical experience in practice, preferably in long-term care, skilled nursing, or assisted living
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions
  • Access to reliable transportation and availability to work 24 hours per month, with flexibility to work off-hours

Preferred Qualifications:

  • Clinical experience in long-term care, skilled nursing, or assisted living
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills, including the ability to document medical information with electronic medical records
