Key Responsibilities:
  • Participate in ongoing interdisciplinary assessments of patients to identify their unique needs and develop personalized care plans.
  • Conduct comprehensive assessments of patients to determine their healthcare requirements and create effective care plans.
  • Collaborate with patients, families, and caregivers to set and achieve mutually agreed-upon goals and objectives.
  • Provide ordered medical services and interventions to patients in their homes, ensuring their safety and well-being.
  • Counsel and educate patients and their families on healthcare-related matters, promoting independence and self-care.
  • Initiate preventative and rehabilitative nursing procedures to prevent complications and promote recovery.
  • Refer patients to other services as needed to ensure comprehensive care.
  • Monitor and evaluate patient outcomes, making adjustments to care plans as necessary.
  • Communicate effectively with healthcare teams, physicians, and other stakeholders to ensure seamless care coordination.
Requirements:
  • Graduate of an accredited school of nursing with a current RN license in SC or compact license.
  • At least 2 years of nursing experience, with 1 year in home health.
  • Proficient computer skills and knowledge of workplace software.
  • Ability to work well under stress, use clinical judgment, and be creative in problem-solving.
  • Valid driver's license and proof of insurance.
