This position requires collaboration with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention.
Key Responsibilities:
- Provide patient and family advocacy to ensure their needs are met and their voices are heard.
- Coordinate discharge planning to ensure a smooth transition from hospital to home or community.
- Develop and implement psychosocial interventions to promote patient and family wellness.
- Collaborate with the clinical team and medical provider to ensure comprehensive care.
- Identify and connect patients and families with community resources to support their needs.
Requirements:
- Master's in Social Work (MSW) required.
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred.
- 3-5 years of experience in social work or a related field.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
