Job Summary

The Registered Nurse is responsible for planning, organizing, and directing home care services. This role requires a strong emphasis on community health education and experience.

Key Responsibilities
  • Conduct initial assessments of patients and families to determine home care needs.
  • Provide comprehensive physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs and initiate necessary revisions to the care plan.
  • Develop and implement individualized care plans that establish goals based on nursing diagnoses.
  • Administer medications and treatments as prescribed by physicians.
  • Counsel patients and families on meeting nursing and related needs.
  • Provide health care instructions to patients as appropriate.
  • Identify discharge planning needs and implement prior to patient discharge.
Communication and Collaboration
  • Prepare daily clinical notes and update primary physicians as necessary.
  • Communicate with community health-related persons to coordinate care plans.
Additional Requirements
  • Participate in on-call duties.
  • Ensure arrangements for equipment and necessary items and services are available.
  • Instruct, supervise, and evaluate home health aide care provided.
Qualifications
  • Current licensure as an RN through the State Board of Nursing.
  • Current CPR certification and valid driver's license.
  • At least one year of general nursing experience, preferably in home health, clinical, surgical, or critical care.
  • Self-directed with good organizational skills and excellent observation, verbal, and written communication skills.
