Job Summary

We are seeking an experienced Physician to join our medical group in Fort Wayne, IN. As a Hospitalist Physician, you will be responsible for providing high-quality patient care in a post-acute hospital setting.

Responsibilities
  1. History and Physical Exams: Conduct thorough examinations of patients and review their medical records, pertinent documents, imaging, labs, and tests.
  2. Diagnostic Testing: Order and study diagnostic tests, including blood tests, X-rays, etc.
  3. Rehabilitation Care: Review and approve rehabilitation care plans as needed.
  4. Interdisciplinary Team Collaboration: Actively participate in interdisciplinary team and patient care conferences.
  5. Patient Care: Diagnosis, management, and treatment of acute episodic illnesses and stable chronic conditions, evaluation of current therapies, health maintenance, health promotion, illness prevention education and counseling, and management of urgent care matters.
  6. Medication Management: Prescribe medications as needed.
  7. Referrals and Consultations: Write referrals and consult other physicians for comprehensive patient care.
  8. Record Keeping: Maintain accurate records and schedules.
  9. Patient Education: Inform patients about their health conditions and prescribed medications.
  10. Health Promotion: Educate patients and their family members on diseases, epidemics, and prevention.
  11. Team Collaboration: Collaborate with the healthcare team to monitor, evaluate, and communicate relevant data regarding clinical problems.
  12. Overseeing Patient Care: Oversee patients' overall health, identify opportunities for each patient's needs, and monitor progress.
  13. Compliance: Identify and address potential deterrents to compliance with treatment regime.
  14. Professional Development: Advance professional knowledge and conduct research.
  15. Follow-up Care: Follow up with patients if they are discharged to assess if needs are being met and determines if additional follow-up care is required.
  16. Operational Policies: Collaborate closely with the leadership regarding operational policies and procedures that may impact patient care.
Requirements
  1. Medical License: Valid Medical license in IN.
  2. Board Certification: MD/DO BC/BE Hospitalist (Internal Medicine OR Family Practice/Primary Care with Hospital Medicine experience preferred).
  3. DEA Certificate: Federal DEA and possess or eligible for Controlled Substance Certificate.
  4. ACLS Certification: ACLS Certified.
