The Edward M. Kennedy Community Health Center is seeking a skilled Family Nurse Practitioner to provide comprehensive primary care to patients in an outpatient environment.
Key Responsibilities- Deliver direct primary care and preventative services to assigned patients.
- Develop therapeutic plans of care for prevention, management, and maintenance of good health.
- Provide health education and counseling to patients.
- Assess, diagnose, treat, and monitor acute and chronic illnesses.
- Collaborate with multidisciplinary teams to manage patient care.
- Follow established practice standards and current evidence-based clinical practices.
- Provide prenatal and postnatal care, as well as family planning services.
- Consult with physician partners and refer patients to subspecialty care as needed.
- Maintain and update Electronic Health Records (EHR) in a systematic format.
- Ensure patient confidentiality and HIPAA compliance.
- Participate in trainings, meetings, and quality improvement projects.
- Provide clinical supervision and mentoring to students.
- Master's Degree as a Family Nurse Practitioner.
- Active, Unrestricted Nurse Practitioner License in Massachusetts.
- 3+ years of experience as a Nurse Practitioner in a related field to Primary Care.
- $105,000-$115,000/yr.
- Qualifies for the HRSA Loan Repayment Program.
