Nurse Field Representative Job Description

We are seeking a highly skilled Nurse Field Representative to join our team at LHC Group. As a Nurse Field Representative, you will be responsible for conducting in-home assessments to determine the level of care required by claimants.

You will need to have excellent critical thinking skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:

  • Conduct comprehensive assessments of claimants' medical history, current health status, and functional abilities
  • Use point-of-care technology to document assessment results in real-time
  • Develop and implement personalized care plans to meet the unique needs of each claimant

Requirements:

  • Current, unencumbered RN licensure in the state of practice
  • Minimum 2 years of home health experience or equivalent experience serving older adults
  • Ability to work effectively in a team environment

Compensation: $65,000 - $80,000 per year, depending on experience
