Job Summary

The RN Unit Care Coordinator is a key member of the Life Care Center of Westminster team, responsible for overseeing the delivery of high-quality patient care on our geriatric nursing unit. This role requires a strong clinical background, excellent leadership skills, and a passion for providing exceptional patient experiences.

Key Responsibilities
  • Supervise and coordinate the daily functions of unit nurses and certified nursing assistants (CNAs) to ensure seamless patient care.
  • Chart patient information accurately, thoroughly, and in a timely manner to maintain up-to-date records.
  • Develop and implement patient care plans in collaboration with the interdisciplinary team to promote optimal patient outcomes.
  • Administer medications as ordered by physicians and respond to emergency situations with a focus on patient and staff safety.
  • Coordinate patient transfers and discharges, ensuring a smooth transition of care.
  • Provide education and training to nursing staff on best practices, policies, and procedures.
  • Maintain accurate records and reports, including patient satisfaction surveys and quality improvement initiatives.
Requirements
  • RN licensure in good standing and current CPR certification.
  • One year of geriatric nursing experience, with a strong background in patient care coordination and leadership.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work in a fast-paced environment, with a focus on providing exceptional patient care and service.
