Clinical Documentation Specialist

We are seeking a highly skilled Clinical Documentation Specialist to join our team at Oak Street Health. This role is an excellent opportunity for individuals who are passionate about delivering high-quality clinical documentation and supporting primary care providers in their daily work.
About the Role
In this position, you will be responsible for documenting patient encounters in a structured note, including the history of the present illness, assessment, plan, and physical exam. You will also be assigning appropriate CPT and ICD-10 codes, preparing After Visit Summaries, and consulting with providers to ensure accurate and specific documentation. Additionally, you will request and review medical records, leverage population health tools to support clinical documentation improvement, and prepare for and support Daily Huddles and Clinical Documentation Reviews.
Required Skills and Qualifications
To be successful in this role, you should have a strong understanding of medical terminology and common medications, either from a pre-medical degree or prior clinical experience. Prior clinical experience, including shadowing and/or volunteering, is strongly preferred. Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve is required where indicated. Advanced listening and communication skills, strong computer literacy, and ability to learn new technical workflows are also essential.
Benefits
Oak Street Health offers a competitive salary of $45,000 - $55,000 per year, depending on experience. We also offer paid vacation, sick time, and investment/retirement 401K match options, as well as health insurance, vision, and dental benefits.
