The Physical Therapist will assess, plan, and implement rehabilitative programs to improve mobility, relieve pain, and increase strength for patients with disease or injury.
Key Responsibilities- Collaborate with physicians to establish and evaluate treatment programs, plans of care, goals, and discharge plans for patients.
- Establish realistic discharge plans incorporating patient and/or family member goals.
- Implement physical therapy treatment plans, select appropriate treatment activities, and revise goals/plans of care with patient/family input.
- Provide ongoing patient/family education and training, discharge instructions, follow-up, and referrals to community resources as needed.
- Document evaluations, plans of care, and treatment sessions according to departmental guidelines and protocols.
- Graduate from an accredited physical therapy program.
- Bachelor's, Master's, or Doctorate degree in Physical Therapy.
- Current certifications in Basic Life Support (BLS) and Physical Therapist (PT) licensure.
- Effective communication and customer service skills.
- Team-oriented and basic computer skills.
