POSITION SUMMARY:

The Speech Language Pathologist is responsible for providing therapeutic services to residents referred by their healthcare providers. This role involves identifying individuals who may benefit from speech therapy through diagnosis, observation, or staff recommendations, and subsequently requesting necessary evaluations and treatment orders.

KEY RESPONSIBILITIES:

  1. Evaluates referrals for speech therapy, interprets physician's orders, establishes treatment goals, designs therapy programs, and implements interventions tailored to the resident's diagnosis and rehabilitation potential. Collaborates with physicians by providing recommendations and progress updates as required.
  2. Ensures accurate documentation in medical records, including evaluations, progress reports, discharge summaries, and treatment logs. Contributes to care planning, Minimum Data Set (MDS) assessments, and quality assurance processes.
  3. Stays informed about advancements in the field to enhance patient care and outcomes. Utilizes evidence-based practices and seeks continuous professional development opportunities.
  4. Engages with the interdisciplinary team regarding patient management and consults with families as necessary. Acts as a consultant to various departments, providing in-service training as needed, and may undertake additional responsibilities as directed by the department head.
  5. Employs effective communication strategies within the rehabilitation team and across departments to foster collaboration.
  6. Maintains a productivity level of 85% or higher.
  7. Performs other duties as assigned by the supervisor.

WORK SETTING:

This position is conducted in a clinical rehabilitation environment and within patient living spaces, necessitating interaction with staff, healthcare providers, residents' families, and the public. The role requires mobility for up to 95% of the workday and the ability to lift, push, or pull weights of 50 pounds or more. Manual dexterity, along with corrected vision and hearing, is essential.

REQUIREMENTS:

The role requires knowledge of speech therapy principles typically acquired through an accredited university program in speech therapy. A valid New York State license to practice as a Speech Language Pathologist is mandatory. Candidates must possess the ability to gather relevant information from patient records and communicate effectively with patients and their families regarding treatment instructions. Additionally, the ability to recognize changes in patient conditions and relay this information to appropriate staff is crucial.
