Job Summary

The Care Partner is a vital member of our healthcare team, working under the direction of the Registered Nurse to provide exceptional patient care and support. This role requires a strong focus on promoting a positive patient experience, partnering with the nurse, physician, and other healthcare providers to deliver excellence in patient care.

Key Responsibilities
  • Provide patient care under the direction of the RN, focusing on promoting a positive patient experience.
  • Assist with daily living activities and comfort measures, ensuring the patient's physical and emotional needs are met.
  • Perform basic patient care functions, including ADLs, ambulation, and vital signs, while maintaining a safe and clean environment.
  • Participate in hourly patient rounding, answering telephone and call lights promptly and professionally.
  • Resolve patient concerns or escalate to the charge nurse if unable to resolve.
  • Proactively offer assistance and comfort measures to patients and families.
Requirements
  • High school diploma or equivalent required.
  • Basic Life Support (BLS) certification required upon hire.
  • Ability to follow detailed directions and work individually and as part of a team.
  • Strong interpersonal skills and clear verbal and written communication.
Preferred Qualifications
  • Certified Nursing Assistant (CNA) or Patient Care Technician (PCT) certification preferred.
