We are seeking a highly skilled Medical Professional to join our team at SCAN Health Plan. This is a unique opportunity to make a difference in the lives of our members by providing comprehensive care and coordination services.

As an Advanced Practice Clinician, you will work closely with our case management team to ensure timely access to necessary services and collaborate with Primary Care Physicians to provide disease prevention, health promotion, and chronic disease management.

Your primary responsibilities will include:

  • Providing care to assigned members through home visits, telephonic triage, and assessments;
  • Identifying patient health care needs and developing comprehensive plans of care;
  • Collaborating with healthcare professionals to promote shared decision-making and continuity of care;
  • Actively monitoring and addressing clinical quality gaps in care; and
  • Documenting member interactions and visits in accordance with established protocols.

To be successful in this role, you will require strong interpersonal skills, a change agent mindset, and the ability to work independently. You will also need to maintain HIPAA requirements and adapt to new technology and processes.
