Job Title: PCS Assessor Registered Nurse

Vaya Health is seeking a highly skilled PCS Assessor Registered Nurse to join our team. As a PCS Assessor Registered Nurse, you will be responsible for conducting needs-based assessments and eligibility determinations for Medicaid beneficiaries in our region.

Key Responsibilities:
  • Conduct assessments to determine eligibility for PCS services
  • Work with members, family members, and healthcare professionals to schedule assessments
  • Provide education and assistance to members in selecting PCS providers
  • Submit service authorization requests on behalf of PCS providers
  • Ensure assessments are conducted within established timeframes
Requirements:
  • Bachelor's degree in Nursing or other healthcare field
  • Active licensure in North Carolina as a Registered Nurse (RN)
  • Two years of experience in home care setting
  • Experience conducting PCS assessments highly preferred
