Position Overview

Key Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs concerning mobility, endurance, pain, and other medical conditions. Based on these evaluations, the Physical Therapist will:
  • Develop and implement a tailored treatment plan
  • Perform interventions aimed at enhancing the patient's overall well-being
  • Monitor and assess the patient's progress

Additionally, you will:
- Evaluate the patient's functional capabilities and physical therapy requirements
- Collaborate with physicians to formulate the therapy plan of care
- Observe and report on the patient's response to treatment and any changes in their condition to the supervising nurse or physician
- Provide treatment to alleviate pain, restore function, and maintain optimal functioning using established standards of practice
- Educate and inform healthcare team members, patients, and their families about the care plan, functional limitations, and progress towards goals.

Qualifications

Licensure Requirements:
- Current Physical Therapy licensure in the state
- Current CPR certification
- Valid driver's license and vehicle insurance, with access to a reliable vehicle
