About the Role

The Registered Nurse is a critical member of our team, responsible for administering skilled nursing care to patients requiring intermittent visits in their homes. You will work closely with patients, families, and healthcare providers to develop and implement personalized care plans that meet the unique needs of each individual.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring

Requirements

To be considered for this role, you must have:

  • A current, valid Pennsylvania license as a Registered Nurse
  • A minimum of two years of experience as a professional nurse in a medical/surgical work area and one year of home health experience (preferred)
  • Excellent critical thinking, communication, and collaboration skills
  • Reliable, independent means of transportation to make home visits
