GIMAG Transitional Care is a 6-bed Congregate Living Health Facility dedicated to providing exceptional care for medically fragile patients requiring sub-acute services. We are currently seeking Full-Time Licensed Vocational Nurses (LVNs) and Licensed Practical Nurses (LPNs) to join our compassionate team.
Position Overview:
As a vital member of our healthcare team, you will be responsible for delivering and supervising patient care with a focus on safety and quality. Your expertise will be essential in managing the needs of our residents.
Key Responsibilities:
- Monitor, document, and report on patient conditions effectively.
- Administer medications as per physician directives.
- Record vital signs including temperature, respiration, and pulse.
- Prepare treatment trays and necessary equipment for procedures.
- Maintain a thorough understanding of each assigned patient's health status.
- Review shift reports to identify changes in patient conditions and delegate tasks to nursing assistants.
- Ensure accurate record-keeping and provide updates to incoming nursing staff.
- Notify the Director of Nursing Services and/or Administrator regarding any significant changes in patient conditions or operational issues.
Qualifications:
- Current California nursing license in good standing.
- Minimum of 1 year experience in long-term care preferred.
- CPR and First Aid certification required.
- Excellent communication skills and a professional demeanor.
