Job Title: Peer Specialist

Family Service League Inc. is seeking a Peer Specialist to join our team at the Center of Treatment Innovations in Hauppauge. As a Peer Specialist, you will provide in-person and telephone-based support, coordination of care, and linkages in the community and onsite at the 24/7/365 Crisis Stabilization Center and an Outpatient SUD program.

Responsibilities:
  • Provide recovery-based support to facilitate engagement in assessment and services.
  • Work as part of a multidisciplinary team providing crisis assessments in the community, via telehealth and onsite at the crisis stabilization center.
  • Provide warm hand off linkage to aftercare and follow up with clients to monitor engagement.
  • Complete all documentation in an electronic medical record (EMR).
  • Coordinate with outside providers, collateral contacts, and other stakeholders.
  • Attend scheduled community events and engage in community outreach to individuals in need of support and services.
  • Participate in routine supervision/team meeting.
  • Complete training and maintain proficiency with de-escalation and techniques for crisis intervention.
  • Establish and maintain positive working relationships with others both internally and externally to achieve the goals of the organization.
  • Communicate effectively, both verbally and in writing.
  • Foster teamwork, work cooperatively and effectively with others to set goals, resolve problems, and make decisions.
  • Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.
  • Set priorities, develop a work schedule, monitor progress towards goals, and track details, data, information, and activities.
Qualifications:
  • High school diploma or equivalent required.
  • Peer Certification, Provisional Certification (CRPA, CRPA-P), or willingness to become certified required.
  • Prior experience as a peer, paid, or volunteer preferred.
  • Relevant life experience required and willingness to draw upon this experience to engage individuals and serve as a role model required.
  • Proficient computer skills, including Microsoft Office required; experience with EHR preferred.
  • Bilingual in Spanish preferred.
  • Valid and clean NYS Driver's License required.
