Since its inception, Boston Health Care for the Homeless Program has been dedicated to providing equitable and dignified access to high-quality healthcare for individuals and families facing homelessness in the greater Boston area. Each year, we serve over 10,000 homeless individuals, ensuring they receive comprehensive healthcare services ranging from preventive dental care to advanced cancer treatment. Our team of clinicians, case managers, and behavioral health professionals operates across more than 30 locations, catering to some of the most vulnerable members of our community.

Position Overview:

Work Schedule: Full-time; 40 hours per week, with the flexibility to work 1-2 early evenings weekly. Rotating on-call coverage for Family Team patients is required.

Boston Health Care for the Homeless Program is seeking a skilled Family Nurse Practitioner/Physician Assistant (NP/PA) to become part of our Family Team, an innovative initiative delivering comprehensive medical services to families and youth/young adults experiencing homelessness. Our Family Team is dedicated to breaking the cycle of intergenerational homelessness by addressing both acute and chronic medical and mental health needs through a holistic care model that simplifies access to healthcare for those facing numerous barriers.

In this role, the NP/PA will function as a mid-level provider within the Family Team, responsible for conducting health assessments for families, pregnant individuals, and youth at various locations. Responsibilities include individualized care management focused on health promotion, disease prevention, and risk factor intervention, as well as providing episodic healthcare as necessary. The NP/PA will also implement comprehensive primary care for patients who consider BHCHP their medical home.

Key Responsibilities:

  • Deliver episodic and primary care to individuals in family and youth shelter programs and at our Family Clinic.
  • Collaborate with site case managers to create, implement, and assess individualized care plans based on a collaborative needs assessment.
  • Assist families, pregnant women, and youth in accessing primary care and other healthcare resources, ensuring smooth transitions of care.
  • Educate patients on health promotion and disease management; conduct necessary health screenings and developmental assessments for children.
  • Engage in data collection and analysis for grants and advocacy purposes; support educational outreach by hosting students and residents.
  • Work collaboratively with Family Team members to develop and evaluate program goals and health education materials tailored to the population served.
Qualifications:

  • Must be licensed in the Commonwealth of Massachusetts.
  • At least 1 year of post-graduation experience from an NP/PA program; pediatric experience is essential.
  • Experience working with underserved populations is required.
  • Ability to travel to multiple sites; reliable transportation is necessary.
  • Preferred experience includes adult outreach and LARC.
  • Buprenorphine waiver is a plus.
  • Bilingual proficiency (Spanish or Haitian Creole) is preferred.
  • Understanding of the diverse needs of individuals and families experiencing homelessness, including substance use disorders and trauma-informed care.
  • Willingness to learn new clinical skills to adapt to the evolving needs of the population served.
