Home Health Nurse Specialist

We are seeking a skilled Home Health Nurse Specialist to join our team in Plymouth. As a Home Health Nurse Specialist, you will provide high-quality clinical services to patients in their homes, developing and revising individualized plans of care in accordance with federal and/or state program requirements.
The ideal candidate will have a minimum of one year experience as an RN and current licensure in the state of practice. You will also have the ability to build trusted nurse-patient relationships and work effectively with other healthcare professionals.
This is a rewarding opportunity for a Registered Nurse who wants to strengthen their experience and make a meaningful difference in the lives of patients and families. The estimated salary for this position is $85,000 per year.
Job Responsibilities:- Provide high-quality clinical services within the scope of practice and infection control standards
- Develop and revise individualized plans of care according to federal and/or state program requirements
- Ensure patient eligibility and medical necessity for services as defined by payer source and agency policy
- Current RN licensure in the state of practice
- Minimum one year experience as an RN
- CPR certification
