Job Title: Registered Nurse Case Manager

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. As a key member of our healthcare team, you will be responsible for managing a defined caseload of patients, providing direct patient care, and delivering comprehensive nursing services to individuals in the community.

Job Responsibilities:
  • Manage a caseload of patients, providing direct care and support to ensure optimal health outcomes.
  • Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
  • Collaborate with healthcare professionals, families, and caregivers to ensure seamless care coordination.
  • Conduct thorough assessments, identify areas of concern, and develop strategies to address them.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Document patient progress, outcomes, and any changes to care plans.
Requirements:
  • Valid nursing license in the state of New Hampshire.
  • 2+ years of experience in skilled care, preferably with a focus on case management.
  • 1+ year of experience in nursing supervision or leadership.
  • Recent wound care experience.
  • Ability to lift 50 pounds with no restrictions.
  • Current CPR certification.
  • Reliable transportation.
