Job Title: Social Worker

We are seeking a skilled Social Worker to join our team at Carle Foundation Hospital. As a Social Worker, you will play a vital role in providing social work services to patients and families, addressing their psychosocial needs, and facilitating timely discharge.

Key Responsibilities:
  • Investigate cases of uninsured or under-insured patients to identify available financial resources and government entitlements.
  • Evaluate patients for abuse or neglect and make appropriate referrals.
  • Assist patients in obtaining financial resources and government entitlements.
  • Provide social work intervention to patients and families.
  • Assess patients for IL Medicaid eligibility and/or Carle Financial Assistance Program eligibility.
  • Complete PASRR assessments to facilitate discharging to extended care facilities.
  • Complete mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors.
  • Arrange transportation for patients to medical appointments and/or home from the Emergency Department/Hospital.
  • Educate patients about community resources to meet their needs.
Requirements:
  • Master's Degree in Social Work or Bachelor's Degree in Social Work.
  • Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) in Illinois.
  • Ability to coordinate multiple projects concurrently and establish priorities in a fast-paced acute care setting.
  • Strong planning and organizational skills.
  • Self-motivated and flexible.
